IN 2020, DARTMOUTH HEALTH BEGAN INTEGRATING SOCIAL DETERMINANTS OF HEALTH SCREENING AND REFERRAL FOR ALL PRENATAL CARE PATIENTS AT THEIR FOUR OB CLINICS IN NEW HAMPSHIRE. WE ARE STUDYING THE UPTAKE AND EFFECTIVENESS OF THIS INTERVENTION

We study characteristics of the intervention at each clinic and document perspectives from providers, patients, and other healthcare professionals throughout the implementation of this screening and referral process. Using this information, we aim to evaluate the feasibility of meeting pregnant people's resource needs during prenatal care, gain greater insight into the integration of social screening in health care visits, and evaluate the prospect of improving pregnancy and birth outcomes among all patients who experience social needs during pregnancy in the U.S.
